General Information

The school maintains a clinic for students. Clinic personnel supervise the administration of medications, assess healthcare needs, and administer emergency first aid when appropriate. When students become ill or injured during the day, they should report it immediately to the teacher and request a pass to the clinic. Clinic personnel will contact parents as situations require.

Immunization Information

Contact your health care provider or local health center to receive any needed vaccinations and to obtain a signed Georgia Certificate of Immunization (Form 3231) with “Complete for 6th Grade” or "Complete fo 7th Grade" marked.

Vaccinate your student early and beat the back to school rush! Immunization forms are not sent automatically by the physician or health center. Please request this. The Shiloh Middle School fax number is 678-442-5144. You may scan and email the form to our registrar, Sherell Wright. Updated immunizations must be submitted prior to the start of school.

Please note that there are no 8th grade immunization requirements.

6th Grade Immunization Requirements

All students entering 6th grade are required to meet the following (no new immunization requirements since 2007):

Have two (2) doses of Measles vaccine, two (2) doses of Mumps vaccine, and one (1) dose of Rubella vaccine or laboratory proof of immunity against Measles, Mumps or Rubella.
Have two (2) doses of Varicella (chicken pox) vaccine or documentation of disease or laboratory proof of immunity.
Is your child up-to-date on their Hepatitis B, Polio, Tdap, Meningococcal and HPV vaccines? Contact your child's pediatrician or health care provider to receive any needed vaccinations and to obtain the Certificate of Immunization (Form 3231).

7th Grade Immunization Requirements

All students entering 7th grade (Students born on or after January 1, 2003) are required to meet the following:

Have one (1) dose of Tdap (tetanus, diphtheria, pertussis) vaccine.
Have one (1) dose of meningococcal conjugate vaccine.

CLINIC CARD

It is imperative that parents complete a clinic card at the beginning of the school year. It must include current address and phone number, medical conditions, emergency numbers, and a valid parent signature. If changes occur during the year, parents should notify the clinic. A clinic card must be on file before students are allowed to check out. Otherwise, the student will be checked out unexcused. The unexcused check-out will be changed to excused upon return of the clinic card.

WHEN TO KEEP YOUR CHILD AT HOME

Fever of 100.4 degrees with symptoms
Nausea, reoccurring vomiting, diarrhea or abdominal pain
Sore throat
Persistent cough
Red, inflamed or discharging eyes
Any open sore oozing fluid or pus
Rash with fever or signs of illness
Suspected impetigo, lice or scabies that is not being treated
Earache
Any other unusual signs or symptoms of illness

MEDICATION

To ensure the safety of all students at our school, the following guidelines should be followed when medications are to be sent to school:

1. All medication (prescription and non-prescription) must be taken directly to the clinic, by a parent, for safe storage.

2. All medications, both prescription and over-the-counter, must be accompanied by an Administration of Medication Request Form. This form is available in the clinic or can be printed from the forms below.

3. All medications must be in the ORIGINAL CHILD PROOF CONTAINER. Prescription medications must be in the labeled prescription bottle. Medications stored in baggies, envelopes, etc., will not be administered.

4. Administration of prescription and over-the-counter medicine (even for a short period of time) is discouraged. Parents should check with their physician regarding the need for medications to be administered during school hours. Medications prescribed for three times daily often can be given before school, after school, and at bedtime.

5. Medications must be picked up at the end of the school year or will be disposed of by the school.

Inhalers may only be carried with the student if the doctor supplies a letter specifically stating that the student "must carry the inhaler at all times." Otherwise, all inhalers must be kept in the clinic. Injections must have the Administration of Medication Request and the Injection Permit Form. Both forms are available by request from the school clinic. All medication not retrieved by the parent by the last day of the school year will be disposed of in accordance with state pharmacy regulations.

Opting out of Scoliosis Screening

Parents and caregivers, if it is your preference, you can use Form 4400 to have your student screened or use the form below to opt-out of in-school screening. Simply download and print the document and return to Dawn Young in the clinic at Shiloh Middle School. Please feel free to reach out directly to DPH if you have any questions or concerns regarding scoliosis screening for this school year.
